1. If option is checked, The report will show On Hold and On Hold Reason fields below the Consumption Site Description line.
2. If option is unchecked, the report should not show the on hold sites details.
3. If On Hold = False, sites that were on hold will NOT display on the report. While If On Hold = True, sites that were on hold will display on the report. If On Hold = Blank/Empty, the site will still display the sites that were On Hold.
4. On Hold criteria should default to False
5. Label of Report Options 'Include Consumption Sites on Hold' should be changed to 'Print On Hold Detail'
6. On Hold site should display based on the following criteria options:
