Add a new field Facility Number to Consumption Site screen > Site tab. Suggest cutting the length of the Sequence field and adding this new field to the right of the Sequence field (see the red rectangle).

# Validation: none (allow NULL and blank)
This field should be hidden by default, and only show when OR has Filing for this TA = Yes in Company Configuration > Motor Fuel Tax Forms area > Tax Authorities grid. When this checkbox is checked, it means the User is filing for Oregon (OR) Motor Fuel Tax, and would use the Oregon Facility Number. If a User does not file for OR MFT, they won't need this field.
