State of IN has changed the SF-900 main form. Detailed changes are below.
# Old Section 4 is now Section 5. The section title's text is changed.
# The new form has a brand-new Section 4
## Line 1: from Section 2 Line 5 (already on current main form)
## Line 2: multiply Line 1 by a Surchage Rate, which is from SF900 Main Form Configuartion. MOre on this later.
## Line 3: from SF 900 Main Form Configuration. More on this later.
## Line 4: Line 2 + Line 3 (if Line 3 is a negative adjustment, then user is expected to enter a negative number. So the calculation here is always Line 2 PLUS Line 3).
# Three lines are impacted in Section 5.
## Line 1: now the calculation needs to include Section 4 Line 4.
## Line 4: follow instructions on the form.
## Line 5: follow instructions on the form.
