Steps:
1. Create Terminal to Location Transport Load. Sample below:

2. Go to Motor Fuel Tax Forms > Tax Authorities and open OR Tax Authority
3. From Form Code 735-1302, select Schedule 5 Gasoline (to Own Bulk Plant) and click Run > Custom Date Range
4. Set Date From and To with the same Transport Load's date and click Refresh
Issue:
Incorrect City and State

Acceptance:
Origin City and State should be pulled from Vendor Supply Point
