Make the following changes to the Add Screen Shortcut screen.
# Rename the screen to "Add Screens"
# Replace the 4 toolbar buttons with OK and Cancel.
# Remove all the tabs (or keep one named "Screens")
# Remove the Layout button in the grid toolbar. (not needed with only 2 columns)
