Steps to replicate:
1. Navigate to Logistics > Maintenance > Forwarding Agents > Forwarding Agents tab.
2. Click New button.
3. Click the Close button
4. Click again the New button from the Forwarding Agents
Issue:
The "Uncaught TypeError" will be displayed on the console upon clicking the New button consecutively

The New Forwarding Identity should be displayed correctly without encountering any error.
