Steps to replicate:
1. Create Vendor > Click Save
2. Go to Entity Type > Select User
3. Fill out User Info > Click Save
Issue#1: At first , alert displayed "The record you are trying to delete is being used. This is set to inactive. After several trying to create again user info, issue #2 already appeared.
