Remove the Adjustment and UOM but leave it on the contract sequence to manually enter. UOM will default from the Item.
Add Index Type combo box with the selection of either Fixed or Origin
Add Vendor that is not editable until Fixed is selected for the Index Type. If user later changes the Index Type to Origin then Vendor and Location is cleared. This field will also be required when Fixed is selected.
Add Location combo which will also be required when Fixed is selected but read only until then.
