Add a "COD" select to the customer credit code. this will be used for cash customers

when COD is selected, the credit limit becomes blank and read-only.
18.3 -> Selecting COD will change credit limit to blank (not 0).
on 19.1 we will keep the credit limit and the indicator for cash only sale is the credit code.
