After sending in the file to the IRS, the user recieved a message about the file format. There were two errors:

Your records are not a fixed length of 750 bytes per record or your record lengths vary within the file. If you use carriage return/line feeds, they must be in positions 749-750.
Refer to Publication 1220 or contact your software company or service provider for further information.
Record sequence number 2 is the first known record containing this error.

Your A record is missing or isn t the second record in the file.
Refer to Publication 1220 or contact your software company or service provider for further information.

So basically, issue one was that not all of the lines were 750 characters long, which in looking at the file, that is correct.
The second issue is that there isn't an A record in the file that is used to have the payers information according to the Publication 1220
